The highest quality of care we provide enables our clients to experience independent living while recovering from a surgery, injury, or illness, managing a serious or chronic disease, or dealing with multiple diagnoses, in the privacy of their homes.

Our patient teaching provides the tools and resources to help control patients’ diseases and elevates patients to partners in their own care as they achieve their health goals and live their lives to the fullest. Medication management is an integral part of our skill set. Home Health Nicely Care Team

Our experienced team of aides, nurses, therapists, and other staffs come with a skill set that is unmatched in delivering intervention, teaching and other services prescribed by a physician and tailored to the needs of each patient.

Home Health Nurse

Case management will be assigned to a home health nurse. He or she manages the care plan in partnership with the interdisciplinary team with doctor and therapist. As it’s often said, the home health nurse is the center of the home health medical team.

Home Health Aid

Home Health Aides support the patient with activities of daily living during recovery and until the patient can independently perform such tasks by themselves. Our Aides help with therapy-driven exercise, and personal care, and grooming needs. They are also very nice people who bring a positive, caring spirit to every visit.

Social Worker

Medical Social Workers are available to help determine needs or assistance you may require. They may also help identify any limitations, barriers, or safety issues that might impede recovery, educate you and your family about available community resources and support, and assist with longer term planning following your discharge from home health services, including advance care planning, health care directives, financial assistance, and transportation. Your social worker provides helpful information and assistance as you navigate the bureaucracy and complexity that are sometimes involved in managing your illness or injury.

Physical Therapist

The physical therapist helps you return to your prior level of function following an illness or injury, or, if not possible, helps you adapt to new ways of moving. Your physical therapists help empower you to prevent ongoing loss of independence by developing a program to keep you active.
